At David Pearce Electrical, our top priority is the safety of our staff, clients, business partners and the communities we work within. For that reason, we have put some extra measures in place to protect against the spread of Coronavirus (COVID-19).

To ensure it is appropriate for us to visit your premises we need your assistance. Please complete this mandatory pre-attendance Coronavirus (COVID-19) questionnaire.

I can confirm that myself and/or all other inhabitants of the property (where David Pearce Electrical will be attending) are not unwell or have any of the official acknowledged Coronavirus symptoms, for example:
  • A continuous cough.
  • A high temperature or fever.
In addition, I can also confirm that the persons at the premises where David Pearce Electrical will be attending:
  • Are not clinically vulnerable to Coronavirus due to a medical underlying condition or old age.
  • Have not been advised by a GP, health official or authority to self-isolate.
  • Have not recently returned from an overseas visit (within the last 14 days).
  • Have not been in direct contact with anyone who has shown the symptoms of Coronavirus or been diagnosed with Coronavirus in the last 14 days.
  • Have not been diagnosed as having Coronavirus.
I agree to:
  • Respect and adhere to the required social distancing needs of the David Pearce Electrical staff. It may be necessary for you or others at your premises to wear PPE (David Pearce Electrical will supply if required) if our required social distancing needs are not met.
  • Avail access for David Pearce Electrical staff to wash their hands as required.
